• If you have tested positive for COVID-19 or have been exposed to COVID-19 in the past 14 days, or have any symptoms of COVID-19 do not enter the office building. Return to your car, and call 989-631-6730 for instructions on how to proceed.
• Only the patient being seen will be allowed entrance into the facility.
• A mask is required to enter the office and must be worn over your nose and mouth at all times. Please wear your own mask if you have one available.
